GLP-1 for weight loss: the exceptions process when coverage is denied
Wegovy, Zepbound, and Saxenda denials usually cite plan exclusions or missing documentation. Here is how to invoke the exceptions process and when an appeal can win.
Plan exclusion vs medical necessity
If the plan excludes anti-obesity medications outright, an appeal will not overturn the exclusion. Check the Summary Plan Description for the exact language.
If the denial is based on medical necessity or step therapy, the exceptions process applies. Most commercial plans require BMI 30+ or BMI 27+ with a comorbidity.
Documentation that wins

ICD-10 comorbidity codes: E11.x (type 2 diabetes), I10 (essential hypertension), G47.33 (obstructive sleep apnea), E78.5 (hyperlipidemia), K76.0 (NAFLD).
Documented prior weight-loss attempts: lifestyle programs, phentermine, orlistat, with dates and outcomes.
Letter of medical necessity citing the STEP trials (semaglutide) or SURMOUNT trials (tirzepatide) for the relevant indication.
Medicare and Medicaid
Medicare Part D historically excludes drugs used for weight loss under 42 USC 1395w-102(e)(2)(A). CMS has indicated possible coverage for cardiovascular indications (Wegovy for MACE reduction) when a separate FDA-approved cardiovascular benefit applies.
State Medicaid programs vary widely; some cover GLP-1s for diabetes only, a minority cover for obesity. Check your state's preferred drug list.

What if I am stable on Wegovy and the plan switches to Zepbound only?+
Request a continuity-of-care exception. If discontinuation would cause clinical harm, the exceptions process at 42 CFR 423.578 (Part D) or your plan's commercial exception rule applies.
Can I get coverage if I use Wegovy for cardiovascular risk?+
After the FDA approved semaglutide for MACE reduction in adults with established cardiovascular disease and obesity, some plans cover for that indication even when weight loss is excluded. Submit cardiology documentation.
